The "flat-rate" system
What is the "flat rate" system ?
- In the “flat-rate” system, a registration contract is signed between the patient, the mutuality and the Medical Center ;
- The contract is individual (not per family) ;
- The contract includes the full membership of the “flat-rate” services of the Medical Center ;

- As a patient you do not pay anything :
- For GP appointments ;
- For medical care given by the nurse in our center
So who pays then ?
- It is the mutuality that pays a monthly lump sum to cover the costs of the care given :
- Your contribution to the mutuality does not change ;
- This is the reason why the contract is signed between three parties: the patient, the mutuality and the medical center.
What do we expect from you ?
- For a better follow-up of your medical file, we expect you to :
- Only consult the general practitioners and the nurse in our centre
- Inform MediSina of all changes in your family (address, telephone number, health insurance, etc.);
- Remain in order with your mutuality.
Note : if you consult another doctor or nurse outside of MediSina, while ours were available, you will not be reimbursed by the mutuality.What does the medical center commit to?
- To take on the health care provided by the GPs and the nurse;
- To organize preventive health activities and health promotion for all patients at the medical center;
- To prepare and maintain a medical record for all patients (accessible to other health professionals; with the agreement of the patient)
How can you make use of this system ?
- Be in order with your mutuality ,
- Habiter dans la zone d'inscription de la maison médicale
- Be registered in MediSina (to have signed the contract).
You always have the possibility to deregister, with prior notice (see the part registration-unsubscribe).
And the other medical benefits ?
- Your mutuality will continue to reimburse you for your visits to specialists (pediatrician, gynecologist etc ..), the dentist, the physiotherapist, as well as the hospital admissions, the laboratory examinations and radiology.

Your mutuality will continue to reimburse the treatments by specialists (pediatrician, gynecologist, etc ..), the dentist, the physiotherapist, as well as hospital admissions, laboratory examinations and radiology.
Registration - Unsubscribe
Who can register ?
The following conditions apply to register in the "flat-rate" system :
- Be in order with your mutual insurance company
- Habiter dans la zone d'inscription de la maison médicale

People who meet the conditions and want to register are asked to make an initial appointment with one of the doctors. This first appointment also makes it possible to receive all relevant information on our operations.
Which documents should I take to register?
- Your ID-card
- 4 stickers of your mutuality
When does it start ?
You will be covered by the flat-rate system from the first day of the month following the signing of the contract (for example = you sign on August 17, 2018, your contract starts on September 1, 2018).
Unsubscribe
As long as you do not let us know anything, you will remain registered at the medical center. To deregister, it is sufficient to sign a deregistration contract at the reception.
When to unsubscribe ?
- If you regularly want to visit another general practitioner or nurse (outside the centre);
- When you switch to a private insurance;
How to unsubscribe ?
- The recommended procedure :
- Call us to let us know that you want to deregister so that we can prepare the necessary documents for deregistration.
- Then, visit us to sign the documents.
- If you have already moved far away (for example abroad) :
- Send us an email at secretariat@medisina.be with the subject « unsubscribe ».
- We will send you the document by email;
- Thank you for signing it and returning it by email as soon as possible.
